Key Responsibilities

As an Experienced Full Stack Data Entry Specialist – Remote Case Management and Care Coordination, you will be responsible for:

  • Conducting comprehensive assessments of individuals' needs and qualifications using care management tools and data/information audits
  • Developing and implementing care plans to address individuals' needs and goals
  • Facilitating and coordinating care plan activities and monitoring progress
  • Identifying high-risk factors and service needs that may impact individuals' outcomes and care planning
  • Collaborating with healthcare professionals, employers, and other stakeholders to overcome barriers to achieving objectives and goals
  • Presenting cases at case conferences to achieve multidisciplinary review and optimal outcomes
  • Recognizing and addressing quality of care issues through established channels
  • Using effective communication and influencing skills to secure suitable choices and services to meet individuals' needs and medical requirements
  • Providing education, information, and support to empower individuals to make informed decisions about their health and well-being
  • Ensuring individuals are knowledgeable and empowered to participate in healthcare decision-making

Essential Qualifications

* Bachelor's degree in a related field (e.g., conduct health, human services, psychology, social work, marriage and family therapy, or counseling)

  • 2+ years of experience in conduct health or social services
  • 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook)

Preferred Qualifications

* Case management and care coordination experience

  • Supervisory experience in conduct health or social services
  • Education and certification in a related field (e.g., conduct health, human services, psychology, social work, marriage and family therapy, or counseling)

Skills and Competencies

* Strong analytical and problem-solving skills

  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong organizational and time management skills
