Role Overview Join Divine Mercy Behavioral Health Center as a Licensed Clinical Social Worker (LCSW) in a remote capacity. In this vital role, you will provide compassionate care and support to patients within our hospice center, ensuring their emotional and psychological needs are met.
Key Responsibilities
- Conduct assessments and develop care plans tailored to individual patient needs.
- Provide counseling and support to patients and their families during challenging times.
- Collaborate with a multidisciplinary team to ensure holistic patient care.
- Document patient progress and adjust treatment plans as necessary.
- Participate in staff meetings and continuing education to enhance service delivery.
Required Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current LCSW licensure in the state of Illinois.
- Experience in hospice, palliative care, or related fields is preferred.
- Strong interpersonal skills and the ability to empower patients and families.
