Job Description MDS Coordinator – RN – Registered Nurse The Carmelite System is a multi-state not-for-profit senior living organization sponsored by the Carmelite Sisters for the Aged & Infirm. The Carmelite Sisters operate a network of 14 skilled nursing facilities throughout the United States. We are seeking a Regional MDS Coordinator. Salary Range $150K to $165K annually based on skills and experience. Regional MDS Coordinator Introduction The Regional MDS Coordinator is responsible for providing oversight, guidance, and support to facility-level MDS Coordinators and interdisciplinary teams across assigned facilities in the Mid-Atlantic region (PA, KY and OH). This position ensures accurate and timely Minimum Data Set (MDS) assessments, compliance with federal and state regulations, and optimization of reimbursement under the Patient-Driven Payment Model (PDPM). Regional MDS Coordinator Qualifications Current Registered Nurse (RN) license, active and in good standing, required. Minimum 3–5 years of experience as an MDS Coordinator in a skilled nursing facility; regional or multi-site experience strongly preferred. Certification as a Resident Assessment Coordinator (RAC-CT) required; RAC-CTA preferred. Ability to travel regularly throughout the Mid-Atlantic region (up to 75%). In-depth knowledge of CMS RAI guidelines, PDPM, ICD-10 coding, and state-specific requirements within the Mid-Atlantic region. Strong analytical skills with the ability to review and interpret clinical, financial, and quality data. Demonstrated ability to provide effective training, coaching, and mentoring to facility staff. Regional MDS Coordinator Job Summary Provide regional oversight of MDS/RAI processes across multiple facilities to ensure accuracy, timeliness, and regulatory compliance. Monitor MDS submissions, validation reports, and correction processes to ensure compliance with CMS regulations. Support facility MDS Coordinators and interdisciplinary teams in accurate coding, case-mix management, and PDPM reimbursement optimization. Conduct regional audits of MDS assessments, care plans, and supporting documentation. Develop and implement corrective action plans in collaboration with facility leadership when deficiencies or trends are identified. Provide training and education to facility-level staff on MDS completion, PDPM, ICD-10 coding, and related clinical documentation practices. Assist facilities in survey preparation and provide support during state and federal survey processes related to MDS/RAI compliance. Collaborate with regional and corporate leadership on quality improvement initiatives, clinical documentation standards, and reimbursement strategies. Monitor and analyze regional MDS data for trends in quality indicators, Five-Star ratings, and compliance risk. Serve as a resource for new facility MDS Coordinators during onboarding and ongoing development. Ensure consistent application of corporate policies and procedures related to MDS/RAI processes.
