The Social Worker will provide comprehensive psychosocial services, including assessment, treatment planning, and coordination of resources, to support veterans' health and well-being. Join our team and help improve the quality of life for those who have served our nation. Duties include but not limited to:

  • Independently perform case finding and outreach activities to identify veterans with psychosocial needs.
  • Screen veterans for mental health, social, and medical concerns to determine appropriate interventions.
  • Conduct comprehensive psychosocial assessments to evaluate the veteran's overall well-being and needs.
  • Develop individualized treatment plans in collaboration with veterans, their families, and interdisciplinary treatment teams.
  • Provide individual, family, and group counseling or psychotherapy to address emotional, behavioral, and social issues.
  • Coordinate community-based services and resources, including referrals to VA programs, government agencies, and community organizations.
  • Identify high-risk patients requiring case management and coordinate services to address complex needs.
  • Offer crisis intervention services, addressing immediate concerns and coordinating follow-up care.
  • Serve as a liaison to family members, facilitating family conferences and providing support during treatment.
  • Maintain effective therapeutic relationships with veterans and their families to promote engagement and trust.
  • Provide consultation and education to VA staff regarding the psychosocial aspects of veteran care and treatment adherence.
  • Evaluate own practice through peer review, case conferences, and organizational assessments to ensure quality care.
  • Communicate effectively, both orally and in writing, with individuals from diverse backgrounds.
  • Facilitate referrals based on veteran needs and eligibility, including benefits and service programs.
  • Support VA missions by delivering respectful, courteous, and customer-focused service to veterans, families, and colleagues.
  • Handle conflicts and resolve problems constructively and professionally.
  • Utilize word-processing, data management, and computer systems relevant to VA documentation and reporting.

Work Schedule: Monday- Friday, 7:30am-16:00pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
